In many cases, proper cooking, cleaning, and food handling dramatically reduce the risk.
The real concern is how certain foods are prepared and where they come from.

Here are four food groups that deserve extra attention.

1. Raw or Undercooked Freshwater Fish

Freshwater fish is one of the best-documented food sources of certain parasitic infections.

According to the World Health Organization, parasites such as Clonorchis and Opisthorchis can infect people who consume raw or inadequately cooked freshwater fish. These infections are particularly associated with certain regions of Asia and Eastern Europe.

Some infections can remain unnoticed for a long time. Chronic infection with certain liver flukes can cause inflammation and damage to the bile ducts, and WHO classifies Clonorchis sinensis and Opisthorchis viverrini as carcinogenic because of their association with bile-duct cancer.

The safest approach is simple: freshwater fish should be thoroughly cooked rather than consumed raw or undercooked.

2. Raw or Undercooked Pork

Pork can be associated with several foodborne hazards, including parasitic infections when meat is inadequately cooked.

One important example is Taenia solium, commonly known as the pork tapeworm. The broader family of foodborne parasitic diseases includes taeniasis and cysticercosis, both recognized by WHO as significant public-health concerns.

Proper cooking is one of the most important ways to reduce the risk associated with undercooked meat.

It’s also important to distinguish between eating pork containing tapeworm larvae and cysticercosis, which occurs when people ingest T. solium eggs, usually through fecal contamination. The two conditions are related but are not the same thing.

3. Raw Watercress and Other Freshwater Plants

A salad may look completely harmless, but certain freshwater plants can carry parasites if they have been exposed to contaminated water.

WHO identifies uncooked freshwater plants, including watercress, as an important route of transmission for Fasciola infections in endemic areas.

Fascioliasis is caused by liver flukes. Depending on the stage of infection, symptoms can include abdominal discomfort, fever, nausea, digestive problems, jaundice, and other complications.

This doesn’t mean ordinary supermarket vegetables are automatically dangerous. The greatest concern involves contaminated freshwater plants and conditions where sanitation and water quality are inadequate.

4. Raw or Undercooked Freshwater Crabs and Crayfish

Freshwater crustaceans can also carry parasites.

WHO reports that Paragonimus, commonly known as the lung fluke, can be transmitted when people consume raw or inadequately cooked freshwater crabs or crayfish.

Infections can affect the lungs and may cause symptoms such as persistent coughing, chest pain, shortness of breath, fever, or blood-stained sputum. In some cases, the parasite can migrate to other parts of the body.

Cooking these crustaceans thoroughly is therefore particularly important in areas where these parasites occur.

The Good News: You Don’t Have to Stop Eating These Foods

The headline may sound alarming, but the science tells a more useful story.

These foods are not inherently dangerous. The risk comes primarily from contamination and unsafe preparation—especially eating certain fish, meat, freshwater plants, or crustaceans raw or undercooked.

WHO recommends improved food hygiene, sanitation, safe water, and appropriate food preparation as important measures for preventing foodborne parasitic infections.

The FDA also recognizes parasites as one category of organisms capable of contaminating food and causing illness. Its Bad Bug Book provides scientific information about foodborne parasites and ways to prevent related illnesses.

What Should You Do?

Instead of avoiding these foods completely, focus on safer preparation:

Cook meat, fish, and freshwater crustaceans thoroughly.

Avoid eating freshwater fish or crustaceans raw in areas where parasitic infections occur.

Wash fresh produce carefully and pay particular attention to freshwater plants.

Prevent raw meat or seafood from contaminating ready-to-eat foods.

Follow local food-safety recommendations when preparing high-risk foods.

If you have persistent abdominal pain, unexplained digestive symptoms, prolonged coughing, jaundice, or another concerning symptom after eating potentially contaminated food, speak with a healthcare professional. Diagnosis of parasitic infections may require specific laboratory or imaging tests.

The Bottom Line

There is no scientific reason to panic and remove these four food groups from your diet. The evidence-based message is much simpler: raw and inadequately cooked foods can sometimes carry parasites, while proper food preparation significantly reduces the risk.

Food safety is about understanding the risk—not being afraid of food.

Most people never think about parasites when sitting down for a meal.
